ISP Flight LH540 from Nairobi (NBO), Kenya to Johannesburg (JNB), South Africa with 17 crew and 140 passengers. Took off from RW 24 with the leading edge flaps retracted, stalled shortly after take off at a height of 100 ft and crashed 1120m past the runway. The flight crew did not follow correctly the checklist for take-off, and there was no warning for this critical condition. First flight : 1970Airframe hrs : 16781 engines : P&W JT9D-7. 59 people died in the crash and 98 injured. location : +-50m 1°20’06.19”S 36°54’20.66”E

ISP Flight PA1736 from New York to Las Palmas with 16 crew and 380 passengers.

Due to bomb explosion at Las Palmas the aircraft was diverted to Tenerife, Los Rodos apr (TCI). Las Palmas was reopened and the Panam 747 backtracked over RW 12. KLM 747-200 turned 180° at the end of the RW. The KLM crew started take-off thinking the PAnam 747 already had left the runway. But the Panam 747 had missed the intersection to leave the RW. In the last moment they saw the Panam 747 on the runway, the KLM 747 tried to lift off to avoid the Panam 747, got airborne but did not gain hight in time and it crashed with its landing gear against the Panam 747. The KLM crew took-off without ATC clearance, due to misunderstand between the crew and the control tower. First flight : 1969. Airframe hrs : 25725 cycles : 7195 engines : P&W JT9D-7A. 335 people died in the crash and 61 injured. See also Boeing 747-200 accident report. Friday 24 February 1989 United Airlines Boeing 747-122 N4713U c/n 19875

ISP Flight UA811 from Honolulu (HNL) to Auckland (AKL) with 18 crew and 337 passengers. 16 minutes after take-off, while climbing between 22.000 and 23.000 ft an explosive decompression occurred. The aircraft was at that moment 97km south of Honolulu. The crew immediately returned to HNL. The forward cargo door had suddenly violently opened in flight, taking with it a part of the upper cabin fuselage. No.3 & 4 engines got damaged along with the flaps. The crew selected to land with a higher approach speed with only 10° flap setting. It made a successful emergency landing at RW 8L, touching down with a speed of around 370 km/hr. Unfortunately 9 people had been ejected from the plane at the sudden decompression and lost at sea. First flight : 1970

Airframe hrs : 58814 cycles : 15027 engines : JT9D-7A. 9 people died in the crash and injured. The failure of the cargo door was determined to be a faulty design. The aircraft was repaired and put in service under new registration N4724U. It was sold in 1997 to Air Dabia, and flew under reg. C5-FBS and later scrapped.

ISP Flight from New York (JFK) to Paris (CDG) with 18 crew and 212 passengers. First flight : 1971 Airframe hrs : 93303 cycles : 16869 engines : JT9D-7AH. All 230

people died in the crash. The investigation revealed that the crash occurred as the result of a fuel/air explosion in the airplane's center wing fuel tank (CWT) and the subsequent in-flight breakup of the airplane.

HZ-AIH - Boeing 747-168B - Saudi Arabian Airlines

ISP Flight SV763 from Delhi (DEL) to Dhahran (DHA) with 23 crew and 289 passengers. 7 minutes after take-off at 14000 ft the aircraft collided with a Kazakhstan Airlines IL-76TD,reg. UN-76435 operating flight KZA1907 from Shymkent with 27 passengers and 10 crew. First flight : 1982. Airframe hrs : 40035 cycles : 14927 engines : RB211-524C2 . All people on board died in the crash, 312 people + 37 in the Il-76TD.
